All-on-4 Dental Implants: What to Know Before You Decide
All-On-4
September 3, 2025
All-on-4 dental implants are one of the most popular solutions for patients who have lost most or all of their teeth. The concept is simple: four strategically placed implants support a full arch of replacement teeth, giving you a complete smile that looks, feels, and functions like natural teeth.
Before you move forward with treatment, though, there are important things to understand about how the procedure works, what recovery looks like, and whether you qualify.
How All-on-4 implants work
Traditional dental implant approaches for a full arch might require six to eight implants per jaw. The All-on-4 technique uses just four. The two back implants are angled at approximately 45 degrees, which maximizes contact with available bone. This often eliminates the need for bone grafting, even in patients who have experienced significant bone loss.
Those four implants act as a foundation for a full arch prosthesis, a complete set of teeth permanently fixed to the implants. Unlike traditional dentures, which rest on the gums and can slip or shift, All-on-4 teeth are anchored securely and stay in place.
Teeth in as little as 24 hours
One of the biggest draws of All-on-4 treatment is the speed. In many cases, the implants are placed and a set of provisional (temporary) teeth are attached the same day. You can walk into the office with failing or missing teeth and walk out with a full, functional smile.
These provisional teeth are designed to look natural and let you eat and speak while your implants heal. The bone fuses around the implants over three to six months through a process called osseointegration. Once healing is complete, your provisional teeth are replaced with a final, permanent prosthesis crafted for optimal fit, function, and appearance.
Who is a good candidate?
All-on-4 implants may be right for you if:
- You're missing all or most of your teeth in one or both arches
- Your remaining teeth are failing and need to be extracted
- You currently wear dentures and want a permanent, fixed alternative
- You've been told you don't have enough bone for traditional implants
The angled placement of the posterior implants means many patients who have been told they lack sufficient bone are actually candidates for All-on-4. That said, every patient is different. A thorough evaluation including imaging is necessary to determine whether this approach fits your specific situation.
What the procedure looks like
The All-on-4 process typically follows these steps:
Consultation and planning
Your prosthodontist evaluates your oral health, takes advanced imaging (CT scans), and develops a detailed surgical and restorative plan. The position, angle, and depth of each implant is mapped out precisely before surgery begins.
Implant placement
On the day of surgery, any remaining teeth are extracted, the four implants are placed, and a set of provisional teeth is attached. The procedure is performed under local anesthesia with sedation options available. Most patients report less discomfort than they expected.
Healing period
Over the next three to six months, the implants integrate with your jawbone. During this time, you'll wear your provisional teeth and follow a modified diet, starting soft and gradually returning to normal foods as healing progresses.
Final restoration
Once the implants are fully integrated, your prosthodontist designs and fabricates your permanent prosthesis. This final set of teeth is crafted for precision fit, natural appearance, and long-term durability. Practices with an on-site dental lab can control every step of this process for the best possible result.
What does All-on-4 cost?
The cost of All-on-4 treatment varies based on several factors: the condition of your jawbone, whether preparatory procedures are needed, the materials selected for your final restoration, and the complexity of your case. The total reflects a comprehensive process that includes implant posts, provisional teeth, follow-up care during healing, and the final permanent restoration.
The investment is significantly higher than traditional dentures, but the outcomes are very different. All-on-4 implants preserve jawbone, don't require adhesives, don't slip, and can last decades with proper care. When compared against a lifetime of denture replacements, relines, and adhesive costs, the long-term economics are often comparable. We provide a detailed cost estimate during your consultation so you know exactly what to expect.
Financing options and payment plans can make treatment more accessible. Insurance coverage varies. Some plans cover portions of the procedure, particularly if tooth loss resulted from injury or disease.
All-on-4 vs. traditional dentures
The decision between All-on-4 implants and traditional dentures comes down to what matters most to you.
- All-on-4 teeth are fixed in place. Dentures can slip and shift.
- Implants stimulate the jawbone and prevent further bone loss. Dentures accelerate it.
- All-on-4 patients can eat virtually anything. Denture wearers often have to avoid certain foods.
- All-on-4 teeth are brushed and flossed like natural teeth. Dentures require removal, soaking, and adhesives.
- All-on-4 restorations can last 15 to 20+ years. Dentures typically need replacement every 7 to 10 years.
Why a prosthodontist for All-on-4?
All-on-4 treatment involves both a surgical component (placing the implants) and a restorative component (designing and fabricating the teeth). A prosthodontist is a specialist trained in both, with three additional years of education beyond dental school focused specifically on complex tooth replacement and restoration.
This matters because the long-term success of All-on-4 depends on more than implant placement. Bite alignment, aesthetics, material selection, and precision fit all affect how the restoration looks, feels, and holds up over time.
